The only problem may be this

DEV_MODEM_LINK_ORIG=ttyS0

The Mandrake installer's guess was that there is a controller chip modem at /dev/ttyS0.
The devfs controlling file for this situation is I expect /etc/devfs/conf.d/modem.conf
To compensate,
# mv etc/devfs/conf.d/modem.conf  /root/
for safe keeping.

Then just repeat the install:
# rpm -i ltmodem-kv_2.4.18_6mdk-8.22a3-???.rpm
If should complete this time. But one minor fix may be needed, a rename:
# mv /etc/devfs/conf.d/ltmodem /etc/devfs/conf.d/ltmodem.conf
as the devfs daemon, devfsd, is now only reading files ending in .conf
Then inform the daemon with:
# killall -HUP devfsd

At this point the modem system should be functional.
Install the wvdial.rpm,  Run as a tutorial:
/usr/share/doc/ltmodem-kv_2.4.18_6mdk-8.22a3/utils/checkout.

   Download  http://linmodems.technion.ac.il/packages/scanModem.gz
   Within a Linux partition only:
$ gunzip scanModem.gz
   It might be necessary to make executable with:
$ chmod a+x scanModem
   before running diagnostics with:
$ ./scanModem
which has additional diagnostics and info, than in the now old 8.22a3 series installers.
 ModemTesting.txt  will be included in the output.
