  Is this a PCMCIA modem?
That chip is a CardBus Controller manufactured by O2Micron !

Meanwhile I fetched those mails from and to you which were sent or cc'ed
to discuss@linmodems.org
  Your ModemData.txt shows Class 0780: 1106:3068
with subsystem 1025:0030
I am amazed by the failure of the test proposed by Marv.
modprobe slamr did not complain that slamr does not exist.
What does /sbin/lsmod show ? Is slamr in the list?
  Look at what I get ! :
[goldberg@phep5 ~]$ dmesg | grep slamr
slamr: module license 'Smart Link Ltd.' taints kernel.
slamr: SmartLink AMRMO modem.
slamr: probe 10b9:5459 SL1800 card...
slamr: mc97 codec is SIL22
slamr: slamr0 is SL1800 card.
slamr: SmartLink AMRMO modem.
slamr: probe 10b9:5459 SL1800 card...
slamr: mc97 codec is SIL22
slamr: slamr0 is SL1800 card.
slamr: SmartLink AMRMO modem.
slamr: probe 10b9:5459 SL1800 card...
slamr: mc97 codec is SIL22
slamr: slamr0 is SL1800 card.

Whatever, and until Marv is back to further help you, can you please
Open a second window in which you run the 3 following commands
/sbin/lsmod | grep slamr to verify that slamr is loaded
/sbin/modprobe slamr if it is not
/usr/sbin/slmodemd to start the driver

And in the first window, if slmodemd does not immediately exit with an
error:
wvdialconf /etc/wvdial.conf

and of course tell us what happens? Does wvdialconf see the modem ???

If you do not have wvdialconf, see
http://linmodems.technion.ac.il/wvdial.html

Also, if slmodem exits immediately with an error message, please do the
following:

/sbin/rmmod slamr
/sbin/modprobe slamr
cat /var/log/messages | grep grabbed
cat /var/log/messages | grep SIL

and tell us if grabbed and/or SIL are found
                                              Jacques
